Instructions

First, complete the red highlighted fields on the New Intake form (1 page, printable - 110KB, online fillable - 410KB) and fax it to (703) 368-8454 so we can verify your information and insurance coverage as well as enter you into our system.

Once all your information and insurance coverage has been verified, we will contact and instruct you to complete the following Information Packet (7 pages, printable - 55KB). When completed either bring it with you to your first visit or fax it to (703) 368-8454 for Manassas or Woodbridge office visits. For visits to our Stafford office please complete the package from the Stafford Family Counseling forms web page and either bring it with you to your first visit or fax it to (540) 658-0855.
